Think of the Cyberknife/Linac/Gammaknife as an electric kettle.
It has a fixed amount of power.

If I have a small tumour (1 Cup of water in the kettle) it takes 30 sec to heat to boiling (13Gy)
If I have a bigger tumour (10 Cups of water in the kettle) it takes 5 Minutes to heat the water to boiling (13Gy.)
10 times the volume = 10 x the energy to heat up the water.

The amount of Grays is really a measure of how hot you make the tumour.
So like the kettle, a big tumour needs a lot more energy to get it to 13Gy versus a small tumour.

The amount of energy it takes to cook the tumour is proportional to the volume.
a 1cm IAC tictac shaped tumour has about 1/70th of the volume of a 3cm golf ball.

Radiation from an atomic bomb is exactly the same, and poses similar risks.
BUT.
With an atomic bomb you will ingest radioactive fall out, and that will live inside you for months or years.
So having your very own mini gammaknife machine working inside you for decades poses a far greater long term risk, if you survive the initial blast.
Elements like Strontium are chemically similar to Calcium but more reactive!. So radioactive strontium 90 will replace the calcium in your bones.
