Physicians closer to home after surgery

I live far enough away from my treatment center that I would like to have someone local to follow up with for post surgical concerns. My last treatment was nearly four years ago and my follow up has shown no tumor. 
Have any of you that were treated far from home been connected to a local physician for "minor" concerns that may be related to treatment?
