Dr. ordered MRI despite no hearing loss—worried

Hi: I’ve had ear problems my entire life, with numerous infections, sinusitis, and allergy-related issues into adulthood. In January 2022, I saw an ENT for worsening tinnitus. He ordered an audiology test, which he said showed typical, symmetrical, age-related hearing loss (I’m a 62-year-old male). He said the tinnitus was related to that. I went back this January for what felt like fluid in my right ear. He said he saw no sign of fluid and ordered another audiology exam, which actually showed slight improvement over the previous year (very slight, but improvement). Then I ended up in urgent care in February for a sinus infection and ear ache, and the doctor saw “lots of fluid in both ears, especially the right.” I was given prednisone and antibiotics and things improved, but did not go away entirely. So I returned to the ENT in March, and he inserted a tube in my right ear drum. That did help drain more fluid, but I still have tinnitus and I sense some hearing loss in that ear (but he did not do another audiology exam). In my follow-up visit, he said I should get an MRI for possible acoustic neuroma. This floored me. My insurance is not great, and this will cost me a lot of money out of pocket. Is this really necessary given that just three months ago I had an audiogram that showed no worsening of hearing, and was almost perfectly symmetrical? Thanks.

Sorry for what you're going through.  Unfortunately, everyone's symptoms leading to AN diagnosis are different.  There are classic symptoms, of course, but sometimes people find AN with really no symptoms at all.

The only way to really know is with a contrast MRI.  You could consider a second opinion, however, if you're really concerned about the MRI cost.  ANs are slow growing and rarely immediately life threatening.  So you'd have time to seek another opinion.

Hearing saved.  Face is fine. Balance pretty darn good most days.
One year follow up MRI showed no tumor. 
Five year follow up showed no tumor, so I'm in the clear.

Burning Tongue, Loss of Hearing & Balance, and Tinnitus led to MRI. Very small AN found on 11/23/2021
While watching and waiting, lost significant hearing. WRS now at 12% (down from 100%). Was fitted with CROS system on 3/7/22.  Stable MRI on 7/29/22
No treatment yet.

I remember the doctor telling me to get an MRI and I thought he was crazy.  But he was right I had an AN. 
Dx 2.6 cm Nov 2012, 35% hearing loss.  Grew to 3.5 cm Oct 2013.  Pre-op total hearing loss, left side tongue numb.  Translab Nov 2013 House Clinic.  Post-op no permanent facial or other issues.  Tongue much improved.  Great result!!

Nate…. I went through the same symptoms for years until I finally went to the emergency room where a young doctor asked me if I had a history of thyroid problems on my family.  Upon checking my level,  they discovered my thyroid scores were extremely low.  That was 35 years ago.  Started me on synthroid and I have not any cold sinus issues since.  I saw 4-6 specialist during my younger days and everyone of them missed this important discovery.  Before I started synthroid,  I had 4 ruptured eardrums,  dozens of cold sinus issues.  Synthroid is the most widely prescribed medication in the world with zero side effects.  I’ve been on it for 35 years.

I’ve told dozens of the employees and friends who suffer from issues like yours and most have benefited from the test and treatment.

Have your thyroid level checked first before you follow any advice….. it made a life changing difference to made throughout my life.
