Have you tried a custom-made ear plug from the ENT's hearing specialist? They have different filters you can put in so you can block out only the amount of noise you want.
For what it's worth, I don't know if I'd ASK my boss to give me monthly reviews. It just seems that it might be inviting trouble. Also, doing a monthly review of someone whose self-esteem is a little low, may be putting your boss in an unusual situation. It's extra work for them, and they have to literally think up things to say, where some of this stuff may go unnoticed. Maybe you aren't making as many mistakes as you think, but you certainly don't want to be put in a position where you think that your EVERY move is being scrutinized. What kind of job do you have? If your boss occasionally points out a goof, does he/she do it in a serious or light-hearted manner? Is he/she initiating this, or is it because you are asking?
People know you had surgery, and if you are working for a good company, and they know you are a conscientious worker, they are going to be patient with you, and they are going to want to help you anyway. Just do your best, and I would try not to dwell on it (maybe easier said than done). Sometimes the more we stress about something, the more mistakes we make. And the more we try to control something, the more out of whack things can get. But try to make sure you are doing something FUN during your off hours. Get involved in something that is FUN for you...maybe try something brand new, or do something you've always wanted to do. It is important to balance the stress with the fun things.
