Capt Deb,

I tried to complete the survey multiple times but first the first microsurgical option copied itself onto the second one which I e-mailed the administrator about and now the second microsurgery copies itself onto the first one! I logged in again and checked. I know it is not the most popular thing to have two microsurgeries but it happens.

Who will be evaluating the survey?

Right side AN (6x3x3 cm) removed in 1988 by Drs. Benjamin & Cohen at NYU (16 hrs); nerves involved III - XII.
Regrowth at the brainstem 2.5 cm removed by Dr.Shahinian in 4 hrs at SBI (hopefully, this time forever); nerves involved IV - X with VIII missing. No facial or swallowing issues.

Eve,
I'm assuming the evaluators will be the ANA's medical advisory board. It's a pretty long list of professionals from across the country. Their credentials are posted on the site. You are right, I don't think the software is prepared to accept information about  2 surgeries. Hmmmm.......

I think I took it. Although I have the memory loss issue (still) after 4 months. It's frustrating. Can I take it again???

I got a good chuckle out of the memory loss part. It had a box to check that read "don't know" when it asked if you had memory loss! Danijake, it asks for a username and password. If you use the same username and password your info will come up as redundant and I imagine they'll discount one of the surveys. I use the same username and password for everything internet.
