I just watched Ghost Town the other day.  Ricky Gervais' character refers to the anaesthesiologist who is no longer working at the hospital where he had a near death experience because the hospital has a strict 'three strikes' policy, "My anaesthesiologist had two strikes against him?!"  Very funny movie if you need a laugh and a new perspective on life.
